New patient visit
We welcome all new patients and offer a complimentary comprehensive evaluation of your oral health followed by a professional dental cleaning with one of our highly trained hygienists. Dr. Orzech and Associates offer convenient daytime and evening appointments as well as office hours one Saturday a month. We request that you arrive 15 minutes prior to your first appointment to complete a new patient questionnaire and medial history form. |
